2. Fill Out Your Information :
- Member Name : Write your full name.
- Date : Enter the date you are filling out the form.
- Address : Provide your current address.
- Email : Include your email address for communication.
- Phone : Enter your phone number.

3. Indicate Your Decision : In the designated space, reaffirm your decision to terminate your membership by writing your name again.

4. Acknowledge Membership Termination : By signing the form, you confirm that your membership will be terminated immediately.

5. Express Gratitude : You may use the space provided to express your appreciation for your time in the community and wish fellow members well.

6. Dependants' Names : If applicable, list the names of any dependants under the age of 21 for whom payments may need to be returned.

7. Sign the Form : Don’t forget to sign the form where indicated.
